Our vision and values

We are a close, collaborative team of doctors, nurses and support staff committed to providing safe,
timely, convenient and evidence-based care. We work in partnership with neighbouring practices, the
NHS and a wide range of health and care professionals to deliver coordinated services and
enhanced support for our patients.

We aim to:

• Provide high-quality, holistic and personalised care, making the best use of available
resources.

• Deliver excellent, evidence-based care for people living with long-term conditions, as close to
home as possible.

• Promote continuity of care wherever possible, particularly for patients who would benefit most
from an ongoing relationship with a familiar clinician or care team.

• Put patients at the centre of decisions about their care through shared decision-making, clear
communication and respect for individual needs, values and preferences.

• Treat all patients, carers and staff with dignity, respect, kindness, honesty and compassion.

the care of our patients.

• Safeguard children and adults at risk by following best practice, national guidance and local

• Work with local practices, our Primary Care Network and wider community partners to
improve the health and wellbeing of the population we serve.

• Contribute constructively to the commissioning and development of services where
appropriate.

• Encourage prevention, healthy lifestyles and participation in recommended screening and
vaccination programmes.

• Listen to our patients and encourage active involvement through our Patient Participation
Group.

• Act with integrity and maintain the highest standards of confidentiality, privacy and
information governance.

• Provide safe, effective and accessible services within a welcoming and appropriate
environment.

• Support the wellbeing, motivation, development and resilience of our whole practice team.

• Use monitoring, feedback, significant-event learning, clinical review and audit to continually
improve the quality and safety of our services.

• Maintain high standards of care through continuous learning, training, reflection and
professional development.
